Check the professional and concern
The Board receives complaints about medical doctors (MDs), osteopathic physicians (DOs), and physician assistants (PAs). It also covers controlled-substance prescribing by certified registered nurse practitioners and certified nurse midwives. Other nursing concerns, hospitals, nursing homes, surgical centers, and separately regulated professions need their respective routes.

How the official source says to file
- Download the Memorandum of Complaint and complete the complaint and medical-record release. Identify the practitioner, patient, treatment dates, factual concerns, and other people involved.
- The form requests patient identifying information, including date of birth and Social Security number. Sign the release and return the original forms to the address printed on them: P.O. Box 946, Montgomery, AL 36101-0946.
- Send copies of supporting records or correspondence, keeping originals. Materials submitted to the Board cannot be returned. Online, email, and fax filing were not verified.

What this route may not provide
The Board investigates licensing violations and can pursue sanctions or lesser measures. It cannot resolve fee disputes, order reimbursement, or award damages. A complaint does not provide immediate intervention in your treatment.

Do not guess about confidentiality
Anonymous complaints are not accepted. You may request identity protection during investigation, but disclosure may become necessary at a hearing. The form explicitly says the complaint is supplied to the physician for a response; request clarification before relying on confidentiality.

Physician complaints: no filing limit stated by the Board
The Board FAQ says there is no statute of limitations for complaints against licensed physicians, but delay can make evidence harder to obtain. A PA filing deadline was not established by this check; ask promptly. Investigations can take months, and no fixed completion date is promised.

What may happen after filing
Staff check jurisdiction, acknowledge receipt, and may seek additional information or a statement. The Board reviews the investigation and may close it, issue a concern letter, seek expert review, continue investigating, or file disciplinary charges. You receive notice of the disposition.
