Check the professional and concern
The Board's consumer instructions cover unsafe or substandard nursing practice, unprofessional conduct, substance abuse, and practice beyond an LPN, RN, or advanced practice license. This route concerns individual licensed nurses. Nurse aide coverage and complaints against facilities are not established by these nursing instructions; ask the Board about the appropriate destination rather than treating every member of a care team as a licensed nurse.

How the official source says to file
- Start on the consumer complaint page and use its Online Consumer Complaint Form link. The instructions were readable, but the linked online form timed out during this check; successful submission was not tested.
- The page also links a fillable Consumer Report of Possible Violation. Provide your name and contact details, the nurse's correctly spelled name and license number if known, employment information, a specific account, relevant documents, and witnesses. Sign and date the attestation.
- The PDF lists P.O. Box 303900, Montgomery, AL 36130, fax 334-293-5201, and a named staff email. Confirm the current delivery method with the Board at 334-293-5200 if the online route is unavailable.

What this route may not provide
The Board describes investigation of Nurse Practice Act violations and possible license discipline. The checked materials do not establish a route for obtaining damages or a refund; no financial recovery is promised by filing.

Do not guess about confidentiality
Anonymous or deliberately misleading complaints are not processed. Separately, the Board says investigations are not public records; this does not mean a named complainant's identity is guaranteed to be withheld from the nurse.

A consumer filing deadline was not established.
The checked consumer instructions do not specify a filing cutoff. Ask the Board promptly about the incident dates. Its investigative flyer describes six months as a performance measure for resolving investigations, with possible delays, not a filing deadline or promised completion date.

What may happen after filing
Investigators collect records and other evidence; Board nursing staff review practice allegations. A Board attorney assesses whether a violation is supported. Dismissal leaves no public allegation record; substantiated cases may proceed through a consent order or administrative hearing.

State the concern you want reviewed, then add a short chronology, best supporting records, factual consequences, and a specific request for review.
