The State Medical Board regulates physicians, osteopaths, physician assistants, and podiatrists. Complaints begin with the Division's Investigations Section. Check the individual's license and identify the profession; complaints about a facility or another professional may need a different agency.

How the official source says to file
- Begin with the Request for Contact form, giving your contact details, the professional's name, incident dates, and a brief factual description.
- Email it to Investigations@Alaska.Gov or mail it to Investigations Section, 550 West 7th Avenue, Suite 1500, Anchorage, AK 99501. The form expressly permits these two methods.
- This is only a contact request. Staff may ask you to complete a complaint package after reviewing it. Ask what signatures and medical-record authorizations that package requires. For help, call 907-269-8124.

What this route may not provide
The Board's published process concerns licensing enforcement, including restrictions, education, probation, suspension, revocation, and fines. Do not assume that disciplinary action will secure compensation or resolve a private payment dispute; ask Investigations whether your particular concern falls within its authority.

Do not guess about confidentiality
The linked investigative-process guidance says anonymous complaints are not accepted. Investigations are confidential, subject to public-record exceptions, and final disciplinary decisions become public. The professional may receive notice of the allegations and requests for records; confidentiality does not mean identity cannot be inferred.
