The Arkansas State Medical Board licenses medical doctors (MDs), osteopathic physicians (DOs), and physician assistants (PAs). Its licensing material lists these professions separately; identify the person and credential in your complaint. A complaint about a medical office should identify the individual practitioner whose conduct concerns you.

How the official source says to file
- Write a letter expressly stating that you wish to file a complaint. Include the practitioner's full name, patient's full name, your full name and return address, details of the complaint, and your signature.
- Mail the signed letter to Arkansas State Medical Board, 1401 West Capitol Avenue, Suite 340, Little Rock, AR 72201; email it to Regdis@arkansas.gov; or fax it to 501-296-1805.
- You may attach supporting documents. Keep a copy of the letter and attachments. Verbal complaints are not accepted. Ask whether a medical-record authorization or representative documentation is needed for your circumstances.

What this route may not provide
The complaint route asks the Board to decide whether practitioner discipline is warranted under the Medical Practices Act. The checked instructions do not establish a patient-compensation remedy. Ask the Board about any request involving refunds, fees, or another remedy rather than assuming discipline will provide it.
