Use the Dental Board of California (DBC) for dentists, registered dental assistants and extended-function assistants; DBC also oversees assistant permits and duties of unlicensed assistants. Use the separate Dental Hygiene Board (DHBC) for hygienists, including alternative-practice and extended-function hygienists. A hygienist working in a dentist’s office still has the separate hygiene regulator.

  • DBC: complete its consumer packet with the professional, patient, incident chronology, subsequent dentists and supporting documents. Sign its records authorization and attach proof if acting as legal representative. Confirm current return options with the Complaint and Compliance Unit at 916-263-2300; the packet gives 2005 Evergreen Street, Suite 1550, Sacramento, CA 95815.
  • DHBC: complete and sign its separate complaint and records-release forms, identify the treating hygienist and subsequent providers, and include supporting copies. Return them to DHBC, 2005 Evergreen Street, Suite 1350, Sacramento, CA 95815. Representatives must supply written authority.

Limits

What this route may not provide

DBC excludes ordinary office administration, fees, billing, insurance coverage, reimbursements, financial compensation and rudeness. DHBC excludes fee disputes, general business practices and personality conflicts. Anonymity

Do not guess about confidentiality

Both boards review anonymous complaints, but missing contact information can prevent investigation. Both forms permit sharing personal information with the subject and other agencies, and disclosures allowed by public-record laws or legal orders. DBC’s release warns the subject may receive a complaint summary and records; DHBC’s release says a complaint copy and records may be provided.
