The Board licenses pharmacists, advanced practice pharmacists, interns and technicians, plus pharmacies, nonresident pharmacies, sterile-compounding pharmacies, hospital pharmacies/drug rooms, clinics and outsourcing facilities. Its complaint guidance covers dispensing errors and pharmacist misconduct even without injury, including counseling, supervision, confidentiality and unsafe pharmacy conditions.

How the official source says to file
- Use the online route linked from the complaint page, or sign and mail the paper packet to Board of Pharmacy, Attention: Complaint Unit, 2720 Gateway Oaks Drive, Suite 100, Sacramento, CA 95833. The page says the PDF cannot be submitted electronically.
- Name the pharmacy, pharmacist if known and others involved. Provide dates, prescribed and received medications, prescription number, harm, counseling and prior contacts. Attach copies, retain originals and keep the medication and container until an inspector advises otherwise.
- Return the signed medical authorization with the paper complaint. A representative signs with their relationship; identify the patient and the records source.

What this route may not provide
Ordinary prices, insurance billing, customer service and over-the-counter product concerns are generally outside this route. The Board identifies an exception for Medicare recipients’ required Medi-Cal pricing at participating pharmacies. Its FAQ directs refund requests elsewhere. A pharmacy complaint does not substitute for review of the prescriber’s conduct.

Do not guess about confidentiality
Information is voluntary, but omissions can prevent investigation or cause rejection. Anonymous acceptance is not expressly established. The FAQ says identity may be withheld in some cases, not all. The packet authorizes record sharing with other government agencies for investigations and proceedings. Formal discipline is public.
