  • Use a separate petition form for each practitioner. Identify the patient, practitioner, incidents, witnesses, other treating providers, and your relationship to the patient; attach supporting copies.
  • Sign the petition and complete the medical-record release. The current webpage requires notarization for complaints against physicians and physician assistants. A legal guardian should provide court documents as the form requests.

Limits

What this route may not provide

DPH action concerns the professional's license, such as a reprimand, practice restrictions, or remedial education. It cannot obtain compensation or recover fees for you.
