DPH's practitioner section lists registered nurses, licensed practical nurses, advanced practice registered nurses, nurse-midwives and nurse aides among its programs. The complaint page addresses individual practitioners; licensed facilities have a separate Facility Licensing and Investigations route. DPH does not regulate office clerical staff through professional licensing. Confirm the professional's credential when completing the form.

How the official source says to file
- Complete a separate petition form for each practitioner. Identify yourself, the patient and professional; explain who did what, when and where; include witnesses and copies of supporting material. Sign and date the form and complete the requested medical-record release.
- Send it to DPH.PLISComplaints@ct.gov, fax 860-707-1916, or mail Connecticut Department of Public Health, Practitioner Investigations Unit, MS#12HSR, P.O. Box 340308, Hartford, CT 06134-0308.
- The webpage's notarization requirement specifically concerns physicians and physician assistants. Do not infer that it applies to a nursing complaint merely because the shared form contains a notary section.

What this route may not provide
DPH's authority concerns the provider's license and cannot produce consumer compensation or recover fees. Routine billing, personality, communication and office-management complaints generally fall outside jurisdiction; the FAQ distinguishes possible fraud, such as charges for services never provided.

Do not guess about confidentiality
Anonymous acceptance is not established in these instructions; the form requests identifying information and a signature. DPH warns that complaints and submitted information may be disclosable under state freedom-of-information law. The medical release also permits records to be provided to the subject practitioner and legal representative.

No general nursing filing cutoff stated.
The checked materials do not establish a nursing complaint filing deadline. Ask DPH promptly about the incident dates. DPH says acknowledgment should arrive within two weeks of receipt and asks filers to contact the unit if it does not; that is not a completion guarantee.

What may happen after filing
DPH acknowledges the complaint and opens an investigation when allegations fall within its jurisdiction. Possible action is administrative, such as reprimand, practice restrictions or remedial education.
