Drug Control investigates pharmacy-law complaints; the Commission of Pharmacy reviews them. Licensing categories include resident/nonresident and mobile pharmacies, pharmacists, interns, technicians and pharmacy clerks. Report prescription errors and pharmacy operations such as staffing, supervision, unlicensed work, impairment, privacy or unannounced closure. For an out-of-state/mail-order pharmacy, include its state; Drug Control can forward the complaint to the appropriate board.

Submission

How the official source says to file

  • Prescription errors: email the form to DCP.RxError@ct.gov, or email the requested information if you cannot complete it. Mail is also accepted at Drug Control Division, 450 Columbus Blvd., Suite 901, Hartford, CT 06103; fax 860-706-1350.
  • Provide patient and complainant details, pharmacy and pharmacist if known, prescription number, prescribed/dispensed medication and strength, fill/discovery dates, chronology, contacts and where evidence is held. Include photographs if available. The checked one-page form does not contain a signature or notarization field.
  • Other pharmacy-operation complaints: follow the pharmacy-specific instruction to email DCP.DrugControl@ct.gov. Ask staff about any further records authorization needed.

Limits

What this route may not provide

The pharmacy instructions exclude medication pricing, fees and billing disputes, and direct copay, prior-authorization and insurance concerns to the Insurance Department. A refused transfer is not automatically misconduct: the page notes legal restrictions. DCP does not act as an individual’s attorney.
