- Use the DELPROS File A Complaint form and select the appropriate profession. Provide your contact details, the subject’s name and location, any known license number, and business details when the pharmacy is involved.
- Describe dates, times, locations and conduct; include the patient’s name and birth date for a health-profession complaint. The form asks about supporting documents and lets you add witnesses. Later signature or patient-release requirements were not established; follow staff requests.
- For assistance or uncertainty about the respondent, call 302-744-4500 and ask for an investigator before filing.

What this route may not provide
DPR excludes ordinary insurance coverage/denials, consumer-fraud or billing disputes, corporate matters and contractual disputes from this licensing intake. Identify a specific pharmacy-practice concern rather than relying on ownership or a financial disagreement alone. A refund or damages remedy was not established by the checked instructions.

Do not guess about confidentiality
