Check the license and concern
Psychologists use the Psychology board; clinical social workers, marriage/family therapists, mental-health counselors and listed interns use their shared professional board. Both connect to DOH complaint intake. Substance-use services can instead involve a Department of Children and Families-recognized certification; that is distinct from a Chapter 490/491 professional license. Its separate credential-complaint procedure was not established. Psychiatrist MD/DO and psychiatric-nurse complaints use medical/nursing guides.

Match the route to the credential
Use the license listed in the practitioner’s records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

- Start at DOH’s Complaint Forms page. Its linked health-care complaint portal asks routing questions and then provides instructions for the appropriate agency. Give the actual professional credential; do not select a licensed therapist merely because an addiction worker provides counseling.
- DOH says complaints against licensed professionals must be written and signed, rather than made by telephone. Prepare practitioner identity, treatment dates and location, a factual narrative and supporting information showing a possible legal violation. Follow the route-specific form instructions.
- Current detailed signature screens, treatment-record releases, notarization and authority-to-file documentation were not established. Ask Consumer Services about required patient consent before supplying another person’s records. Keep a copy and the submission acknowledgment.

What this route may not provide
DOH excludes ordinary billing disputes or rudeness without a practice-law violation. Although complaint guidance says it cannot obtain refunds as ordinary dispute resolution, §456.072(2)(i) permits a disciplinary order refunding collected patient fees. Discipline may also include restrictions, education, fines or license loss; repayment is not guaranteed. Facility/program issues require separate routing.

Do not guess about confidentiality
Under §456.073, anonymous complaints may be investigated when legally sufficient and supported by credible, substantial preliminary evidence. The subject ordinarily receives the complaint. Investigations are generally confidential until ten days after probable cause or waiver; patient-identifying information remains protected as provided by law. Confidentiality does not mean the practitioner cannot identify the patient.

Act promptly: six-year disciplinary limit with exceptions
Section 456.073(13) generally bars DOH from filing an administrative complaint more than six years after the incident. Exceptions include criminal conduct, diversion, sexual misconduct and impairment; qualifying concealment can extend discovery time, capped at twelve years. This is an agency-action limit, not permission to wait six years to report. The website summarizes timing differently using last treatment and investigation completion; ask promptly how it applies. Do not transfer it to private certification or court claims.

What may happen after filing
Consumer Services screens jurisdiction and whether facts show a possible violation. Legally sufficient complaints may proceed to investigation and professional review, closure or disciplinary proceedings. DOH describes notifications at key stages. Ask separately about the process for a DCF-recognized addiction credential.

Prepare a reviewable submission
State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.
