Check the license and concern
The Board’s categories include pharmacists, interns, registered technicians, pharmacy permits and nonresident sterile-compounding permits. DOH Consumer Services receives complaints concerning regulated people and establishments, including unlicensed activity. Identify the individual and dispensing pharmacy where relevant, with location and permit details; ask staff about jurisdiction for a particular out-of-state transaction.

How the official source says to file
- Follow the DOH complaint portal linked from the official page. Provide facts showing a possible professional-law violation, names, dates, the pharmacy location and supporting copies such as records and correspondence. Ordinary complaints must be written and signed; licensed-practitioner/facility complaints cannot be filed by telephone.
- The portal’s later fields, release requirements and alternative paper instructions were not established by this check. Ask Consumer Services for help if the online route is unsuitable.

What this route may not provide
Ordinary billing disputes are generally outside the disciplinary intake. However, section 456.072(2)(i) authorizes refund of fees collected from a patient or third party as a disciplinary penalty. A refund is not automatic, and this is not a general damages proceeding or insurance appeal.

Do not guess about confidentiality
DOH may investigate an anonymous written complaint if legally sufficient, substantial and supported by preliminary inquiry. The subject generally receives the complaint when investigation begins. Investigative material stays confidential until ten days after probable cause, or earlier waiver by the subject; patient-record protections remain.

Six-year administrative-complaint rule; substantial exceptions
Section 456.073(13) generally requires the agency’s administrative complaint within six years of the incident. Criminal conduct, controlled-substance diversion, sexual misconduct and impairment are exceptions. Concealment, fraud or intentional misrepresentation preventing discovery can extend the period, no later than twelve years after the incident. Ask promptly; this is not simply six years available for a patient to report.

What may happen after filing
Consumer Services screens legal sufficiency and may request information. Supported cases can proceed through investigation, probable-cause review and Board discipline; others close.
