- Use the online form linked from the complaint page or complete the downloadable complaint form. Identify the licensee, patient, treatment concerns and supporting facts.
- The paper form requests a signed truthfulness certification, dates, other providers consulted, and a patient-record release. Read the release before signing and provide copies of supporting materials.
- Mail to the Board at 2 Martin Luther King Jr. Drive SE, East Tower, 11th Floor, Atlanta, GA 30334. The form also permits fax to 470-386-6137 and lists complaints.dentistry@dch.ga.gov.
