Submission

How the official source says to file

Start with RICO's filing page and choose Formal Complaint. It separately links an Unlicensed Activity Tip route.
Gather and copy relevant documents, then review, date, and sign the complaint. For a licensed healthcare-provider complaint, include the signed authorization to release health records. The FAQ states there is no filing fee and an attorney is not required.

Do not guess about confidentiality

RICO recognizes anonymous reporting witnesses, who receive no updates. For identified complainants, the respondent receives the complainant's identity and allegations. Supporting information is not generally public, but legal and enforcement disclosures may occur.
