- Use the online form linked from the official complaint page or mail the completed paper report to Complaint Intake Unit, 555 West Monroe Street, 5th Floor, Chicago, IL 60661.
- Provide names, contact details, profession, incident dates, specific concerns and supporting-document copies. Complete a separate report for each profession. Keep originals.
