How the official source says to file
- Use the online complaint form or mail the completed paper form to DPR Complaint Intake Unit, 555 W. Monroe St., 5th Floor, Chicago, IL 60661. Written complaints are required; the paper instructions exclude telephone and walk-in complaints.
- Include the professional's correct name, profession, contact information, incident dates, and specific concerns. Attach copies of supporting records rather than originals; use a separate form for each profession.
- The online narrative is limited to 3,000 characters and rejects URLs. The instructions permit 111-111-1111 for an unknown respondent telephone number and N/A for an unknown street address, with the remaining address fields completed.
