How the official source says to file

• Choose the Licensing Enforcement online form or downloadable PDF, rather than the separate Homeowner Protection form.
• Provide the professional's name, license number and address, patient name and birth date, incident details and relevant documentation. The paper form has three pages and a signature certification under penalty of perjury.
• Mail the signed PDF and supporting documents to Consumer Protection Division, Indiana Government Center South, 5th Floor, 302 W. Washington Street, Indianapolis, IN 46204. The instructions require postal return of the PDF; do not infer fax filing from the contact fax number.

Do not guess about confidentiality

Licensing Enforcement cannot accept anonymous complaints; its page directs anonymity requests to PLA to file on your behalf. Investigations are confidential, but the form explicitly sends a complete copy to the respondent. Its verification authorizes information sharing for disposition and with public enforcement agencies; do not include a Social Security number.
