How the official source says to file
- Choose the Licensing Enforcement online complaint, or complete the three-page downloadable LE form and return it by postal mail.
- Provide the nurse's name, license number, and mailing address; include other contact details if known, the patient's identifying details requested by the form, incident date and location, narrative, and relevant documentation.
- Sign the paper form and mail it with the documents to Office of the Indiana Attorney General, Consumer Protection Division, Indiana Government Center South, 5th Floor, 302 W. Washington Street, Indianapolis, IN 46204. Do not include a Social Security number.

What this route may not provide
Licensing Enforcement may seek action against a professional license. It cannot serve as your private attorney, file criminal charges, or obtain monetary damages for a licensing violation.

Do not guess about confidentiality
The Attorney General's LE form does not accept anonymous complainants and says a copy goes to the respondent. It directs people seeking anonymity to PLA for guidance. Investigative confidentiality therefore does not conceal your complaint from the nurse.

The Attorney General evaluates the report and decides whether to bring supported allegations before the Nursing Board. The Board determines discipline following a hearing or settlement. Outcomes may include reprimand, probation, suspension, or revocation.
