How the official source says to file

• Use the online link on Licensing Enforcement's page, or complete and postal-mail its paper form to Office of the Attorney General, Consumer Protection Division, Indiana Government Center South, 5th Floor, 302 West Washington Street, Indianapolis, IN 46204.
• Find the respondent's name, license number and mailing address; the agency warns that missing identifiers may prevent investigation. Ask intake for help if you cannot locate them. Provide dates and supporting facts, and sign the certification. Review consent to information gathering and disclosure.
• Do not include Social Security numbers. The form warns that providing one constitutes consent to its disclosure under the stated law.

What this route may not provide

The Attorney General represents the State in licensing matters, not the patient, and cannot obtain money damages through this process or bring criminal charges through Licensing Enforcement. A pharmacy complaint does not establish jurisdiction over every insurance, PBM or prescriber dispute.

Do not guess about confidentiality

The Attorney General does not accept anonymous complaints. Its page directs people seeking that option to PLA, which may complain on their behalf; limited information can impair investigation. Although investigation information is confidential, the form says a complete complaint copy goes to the respondent.
