The Board regulates pharmacists, interns, technicians, support persons, pharmacies and other drug-distribution businesses. Its licensing program also lists nonresident pharmacists-in-charge and outsourcing facilities. The complaint form asks for both personnel and the pharmacy involved. Give the actual dispensing location and relevant credentials, especially when a prescription was shipped from elsewhere.

  • Use the Board's linked complaint form. Provide complainant contact information, patient details, the incident month and year, the pharmacy's identity and location, and a factual narrative. Upload supporting documents and complete the signature and truthfulness certification.
  • Applicable complaint rules also permit written or verbal complaints and specify the complainant's name/contact information, licensee and concise allegations. Ask the Board how to use an alternative method and what authorization is needed if you represent the patient.
