How the official source says to file
- Use the linked Complaint Portal, supply as much relevant information as possible, and upload pertinent records.
- Write down the generated complaint number. The form times out after 30 minutes of inactivity; email KBNdisciplinealert@ky.gov with that number to request a link to resume.
- If document uploads fail, the Board permits sending documents by that email, fax to 502-429-3353, or mail to Investigation Branch, Kentucky Board of Nursing, 312 Whittington Parkway, Suite 300, Louisville, KY 40222-5172.

Do not guess about confidentiality
The checked sources do not establish anonymous intake. KBN's brochure says the nurse receives a copy of a qualifying complaint. Investigative details generally remain confidential while unresolved, with stated exceptions; the existence of an investigation may be disclosed. Do not equate that confidentiality with concealed complainant identity.
