How the official source says to file
- Start the online grievance from the Board's complaint page. The entry instructions ask you to complete sections accurately and provide supporting uploads at the end.
- Prepare dates, prescription details, the names of the pharmacist and pharmacy, and documents that support the specific conduct reported. Confirm the later form's signature, patient authorization and representative requirements with the Grievance Coordinator; those details were not established here.
- Keep the confirmation information so you can identify the report in later correspondence. Ask the Board about alternative filing methods if you cannot use the online route.

What this route may not provide
The grievance process addresses compliance with pharmacy law. It is not a request for restitution and does not arbitrate prescription prices. The online entry also excludes ordinary rudeness, customer-service, pricing and billing disputes. Explain any suspected professional or operational violation underlying a financial concern; do not assume every insurance or PBM disagreement qualifies.

Do not guess about confidentiality
The investigation is confidential while open. Once a case is dismissed, closed by final order or settled, the file generally becomes public, subject to exceptions. The subject is notified during investigation. Anonymous acceptance or withholding your identity from that subject was not established.
