The Board accepts complaints about licensed pharmacies, pharmacists and technicians; its credential materials also cover interns and technician candidates. Pharmacy resources include nonresident and compounding operations. Name the individual and dispensing permit holder involved. The Board also permits PBMs performing specified pharmacy-practice activities, so a PBM's role requires a jurisdiction check rather than an automatic exclusion.

How the official source says to file
- Anyone may submit a written complaint; the Board's form is optional. Its instructions permit mail, fax or email. The current form lists info@pharmacy.la.gov and 3388 Brentwood Drive, Baton Rouge, LA 70809-1700. Confirm the current fax destination before using fax.
- Give complainant and respondent contact details, dates, locations, prescription-label information and the events in order. Attach additional pages or relevant documents as needed. Sign and date the form if using it.
- If filing for another patient, ask which authorization or release the Board needs; the checked form does not establish that requirement.

What this route may not provide
The Board excludes ordinary rudeness, customer service, pricing and billing disputes and cannot obtain restitution or money damages for you. A pharmacist's professional decision not to fill a prescription is not automatically misconduct. Describe the suspected violation and distinguish it from an insurance payment or customer-service disagreement.

Do not guess about confidentiality
The form asks for your identity and signature and expressly allows the subject to request and obtain a copy of the complaint. Anonymous acceptance and broader public-record disclosure were not established; do not assume secrecy from the respondent.
