- Start with File Complaint Now on LARA's complaint page and choose Dentistry in MiPLUS. The page provides account-registration help.
- Include the professional's name and credential, dates, locations, detailed concerns, supporting records, bills or claims, and contact details for witnesses and later treating providers.
