The Board reviews complaints within its jurisdiction involving pharmacists, technicians, interns and pharmacy businesses, as well as specified manufacturers, wholesalers and distributors. Its pharmacy licensing page expressly includes nonresident pharmacies and establishments that prepare, compound or dispense prescriptions. Identify the pharmacy and relevant individual separately in the complaint.

How the official source says to file

  • Complete and sign the current complaint packet; incomplete or unsigned submissions may be returned. The May 2026 form accepts a typed name as an electronic signature.
  • Include the subject's identifying information, incident details, relevant prescription and patient information, a factual statement and supporting copies. The Board may request an additional records release.
  • Send the form to pharmacy.board@state.mn.us, fax 651-215-0951, or mail it to the Board at 335 Randolph Avenue, Suite 230, Saint Paul, MN 55102.

Anonymity

Do not guess about confidentiality

The form treats submitted information as nonpublic or confidential, but authorizes sharing with the Attorney General and use or disclosure as law requires. Withholding identity may prevent investigation and status updates. Choosing email updates waives confidentiality specifically for exchanges through your unsecured email; postal notification is an alternative.
