The Board regulates dentistry and dental hygiene and maintains radiology-permit records. Name all dentists and dental auxiliaries involved, identifying who performed each act. Ask the Board about the relevant permit or supervising dentist when the concern involves an assistant.

How the official source says to file
- Use the electronic complaint wizard, or submit a signed written complaint by email to dental@dentalboard.ms.gov or mail to 715 S. Pear Orchard Road, Suite 200, Ridgeland, MS 39157.
- Give contact details, practitioner and witness names, treatment dates, procedures and a chronology. Include copies of available records, images, subsequent-provider information, bills, insurance explanations and correspondence.
- The checklist requests an Authorization to Release Dental Information with the initial mailing, but the checked checklist contains only one page. Obtain the current authorization from the Board. The online wizard allows attachments at its end.
