How the official source says to file

  • Use the online system linked from Complaint Procedure, or complete the paper complaint and HIPAA authorization.
  • Mail completed forms to MSBML / Investigative Division, COMPLAINT, 805 S. Wheatley Street, Suite 600, Ridgeland, MS 39157; or fax 601-987-6822, Attention Investigations. The form expressly says not to email it.
  • Identify the practitioner, patient, dates, treatment, witnesses and facts. Sign the truthfulness statement and medical-record release. Use a separate authorization for each records provider or facility; someone signing for the patient must attach proof of legal authority. Submit supporting copies and keep originals.
