How the official source says to file
- Prepare a written complaint to the Missouri State Board of Nursing. Rule 20 CSR 2200-4.030 requires the complainant's full name and address.
- Provide enough facts to permit an investigation and explain the conduct believed to violate nursing law. The Board's rule says complaint forms are available on request or through its website; it does not require use of a particular form.
- Confirm the current submission address or delivery method with the Board before sending. The NCSBN member directory lists the Board telephone as 573-751-0681.
- The rule encourages promptly alerting the nurse or facility administration to allow an immediate correction. It states this as an encouragement, not a prerequisite to submitting a complaint.
