How the official source says to file

• Use the secure online complaint link on the Department's current complaint page, or its signed paper form. The webpage permits mail, email and fax; confirm the current fax destination before using it.
• Name the practitioner and profession, provide dates and locations, explain events in order, identify witnesses, and attach copies of supporting records. The paper declaration authorizes use of your name and information to investigate or prove the allegations.
• Mailing instructions conflict: the webpage and November 2024 form give Compliance Unit, PO Box 200514, Helena, MT 59620-0514; the linked complainant FAQ gives Office of Legal Services, PO Box 1728, Helena, MT 59624. Use the online link or verify the address at 406-841-2312. The paper form lists DLIBSDComplaints@mt.gov.

What this route may not provide

Licensing sanctions can include reprimands, fines, restrictions, suspension or revocation. The Department cannot award damages, order a refund or settle a private dispute. A disappointing experience or billing disagreement must involve a possible licensing-law violation for the Board to act.

Do not guess about confidentiality

Anonymous filing is generally unavailable, and some boards prohibit it. Contact the Department if identifying yourself raises concerns about reporting serious harm. The complaint is provided to its subject for response. Investigation material is generally protected from public release, while disciplinary orders are public; testimony may be needed.

Filing deadline not established; review may take months

A filing deadline was not established by this check; ask promptly. The Department says review and investigation may last several months or longer, depending on complexity and workload. Its FAQ expressly says a licensing complaint does not stop or extend court or other legal deadlines.

What may happen after filing

Staff check jurisdiction and gather records, interviews and the practitioner's response. Screening review may dismiss or advance the case to proposed discipline, agreement or hearing. The complainant receives outcome notice but is not a party and cannot appeal a screening dismissal.

State the concern you want reviewed, then add a short chronology, best supporting records, factual consequences, and a specific request for review.
