Pharmacy categories include community, hospital or institutional, limited-service, outpatient surgical-center and out-of-state mail-order operations. Identify the relevant individual and facility. A dispensing registration does not establish Board authority over every aspect of a prescriber's practice.

How the official source says to file
- The Department prefers its linked online complaint route. Alternatively, send the signed paper form to DLIBSDCOMPLAINTS@MT.GOV or Compliance Unit, PO Box 200514, Helena, MT 59620-0514. Confirm the correct number before using the stated fax option.
- Describe what happened, when and where, the subject's identity and license if known, witnesses and how you learned the facts. Include relevant copies and a clear chronology.
- Read and sign the form's truth certification. Signing authorizes Department use of your name and personal information to investigate and prove the allegations.

What this route may not provide
The Department handles licensing violations and unlicensed activity but does not represent either side, settle private disputes or award personal money damages. Available licensing sanctions can include restrictions, fines, suspension and revocation.

Do not guess about confidentiality
The Department generally does not accept anonymous complaints. If identifying yourself presents a serious risk, contact a Compliance Specialist before filing. The subject normally receives the complaint, subject to confidentiality restrictions. Investigation is generally confidential; dismissed matters remain private, while final disciplinary orders are public.
