Check the professional and concern
DHHS Investigations handles licensed or unlicensed health care practice. Nebraska's licensing pages identify M.D. physicians, osteopathic physicians and physician assistants within DHHS professional licensing. Use the public individual complaint route and identify the practitioner's precise credential; do not use a mandatory-report form intended for facilities or licensed professionals.

How the official source says to file
- Follow File Complaint to the public individual/business intake, or download the Public Complaint Form for an Individual.
- Provide the subject's name, profession and license number if known; describe what occurred, where and when, and attach relevant documentation. Identify the patient, sign the accuracy statement, and answer whether you agree to testify at a licensure hearing.
- Written complaints may be mailed to DHHS Division of Public Health Investigations, Attn: POL Investigation Unit, PO Box 94722, Lincoln, NE 68509. The website warns not to email allegations. Call 402-471-0175 to clarify filing or authorization requirements; the listed contact fax alone was not treated as a verified submission instruction.

What this route may not provide
DHHS examines possible professional-law violations. It does not arbitrate fee, insurance or employment disputes, or investigate mere rudeness and personality conflicts. Private legal remedies are separate. The reviewed instructions describe license enforcement rather than a guaranteed compensation process.

Do not guess about confidentiality
The public form offers an anonymous preference while also asking for identifying information and a signature; ask DHHS how to complete those fields if anonymity matters. Complaint and investigation records are confidential. Information can be shared with the subject when authorized or necessary for investigation, but identity is withheld during investigation without written permission. Identity may become public in a contested case.

Delay can affect review; filing deadline not established
A filing deadline was not established by this check; ask promptly. DHHS specifically considers whether a complaint has been delayed too long to justify investigation. Its 30-day mandatory-reporting discussion concerns credentialed reporters, not an established patient filing window. It says thorough investigation takes considerable time without promising a completion date.

What may happen after filing
DHHS assesses jurisdiction and whether evidence supports investigation, including witness availability. Completed findings go to the relevant professional board, which recommends action to the Attorney General. Possible next steps include discipline proceedings, voluntary license limitations or surrender, and an assurance of compliance.
