How the official source says to file

• Select the public complaint form. DHHS also provides distinct routes for mandatory professional reports, self-reports, facilities, and insurers.
• Identify the nurse and profession, license number if known, incident dates and locations, people involved, and the conduct. Attach supporting evidence and provide relevant patient or client information requested by the form.
• The form asks for your contact information, willingness to testify, and a signed certification that the information is true. Review those fields before relying on an anonymity request.
• Use the online form or mail the printable form to DHHS Division of Public Health, Investigations, Attn: POL Investigation Unit, PO Box 94722, Lincoln, NE 68509. DHHS expressly says not to email complaint allegations.

What this route may not provide

DHHS does not arbitrate fee, insurance, or labor disputes, or ordinary rudeness and personality conflicts. Its initial legal review considers jurisdiction, available evidence, delay, and other circumstances; filing alone does not establish that a disciplinary case will proceed.

Do not guess about confidentiality

The public form offers an option to request anonymity while also requesting identifying information and a signature. The investigation page says complainant identity is not disclosed during investigation without written consent, but may become public in a contested case. Ask DHHS how its anonymity option applies before assuming your identity will never be disclosed.

Act promptly; delay can affect DHHS's initial review.

This guide does not establish a numerical public nursing complaint deadline. DHHS expressly considers whether a complaint was delayed too long, so contact it promptly about your circumstances.

What may happen after filing

DHHS screens legal sufficiency, investigates appropriate complaints, and may notify the subject. An investigative report goes to the professional board for a recommendation to the Attorney General. Possible next steps include a disciplinary petition or an agreed regulatory resolution.
