  • For a professional complaint, use the public complaint intake or signed paper form. Provide the person's identity, dates, facts, patient information and witnesses; the paper form asks whether you will testify. Mail to Public Health Investigations, PO Box 94722, Lincoln, NE 68509, or fax 402-742-8335. The agency expressly says not to email allegations.
  • For a facility complaint, use the facility form, call 402-471-0316, fax 402-742-2389, or mail Health Facility Investigations, Licensure Unit, PO Box 94669, Lincoln, NE 68509-4669. Its form requests facility and patient identification, incident details and a narrative.
