How the official source says to file
- Use the English or Spanish online form linked from the complaint page, or complete the paper complaint, verification and records authorization.
- The paper route requires signed, dated declarations, practitioner and patient details, treatment dates, a factual account and supporting copies. The records authorization covers all dates of treatment and permits use in investigation or public hearings and further lawful disclosure.

What this route may not provide
The Board does not resolve fee, billing, insurance or warranty disputes or award civil damages. Discipline may include patient reimbursement in some cases, limited by the webpage to verified personal out-of-pocket costs, excluding insurer or third-party payments and credit interest. The packet also excludes another dentist’s treatment costs, including corrective work. Repayment is not guaranteed.

Do not guess about confidentiality
The practitioner receives an unredacted complaint. Review-panel redaction is not anonymous filing. Read the verification carefully: it requires keeping complaint and investigation materials confidential, restricts their use in civil cases except ordinary pre-existing clinical records, and warns that public disclosure may cause dismissal.

Standard-of-care concerns: treatment less than five years old
The Board says it does not investigate standard-of-care treatment performed five years ago or longer. Other conduct deadlines were not established; ask promptly. Filing does not pause a civil lawsuit deadline.

What may happen after filing
Counsel checks jurisdiction; an investigator reviews records and clinical issues. A panel recommends dismissal, agreement or a formal hearing, with Board approval. Investigation materials are restricted; status emails contain limited details.
