Its applications cover interns, technicians and trainees, and business categories addressing online pharmacies, out-of-state operations and compounding. For a remote pharmacy, identify the dispensing location and ask which Nevada license or permit is involved.

How the official source says to file
- Use the online form linked from the complaint page, or complete and sign the linked paper form. Mail the paper complaint to the Board at 985 Damonte Ranch Parkway, Suite 206, Reno, NV 89521. Call 775-850-1440 for form assistance.
- Identify the pharmacy or company and each relevant pharmacist, technician or other individual. Describe dates and events; include relevant prescription, patient and practitioner information and supporting copies.
- The paper form authorizes Board staff to obtain medical-history records, including pharmacy records, as needed for the complaint. Any limitations on that authorization must be stated in writing. Read the authorization before signing and dating the form.

What this route may not provide
