How the official source says to file
- Plc 204 directs written complaints to Complaints@oplc.nh.gov; Plc 202.10(b) requires emailed documents as PDF attachments. Include the subject's name and license if known, dates, places, facts, witnesses, attempted resolution and relevant contacts with other agencies.
- Include your name, daytime telephone, email and mailing address, plus any attorney's contact details. The rules require signature and date by you or an authorized representative; electronic filing itself supplies the signature and transmission date.
- The filing certifies that the complaint was read, is authorized and supported by grounds, and is not filed for harassment or delay. A current pharmacy-specific form or records-release requirement was not verified.

What this route may not provide
The statutes authorize licensing measures including reprimands, restrictions, fines, suspension and revocation. The checked rules do not establish personal refund, insurance or PBM-dispute remedies. A pharmacy complaint should not be assumed to resolve a separate prescriber's licensing conduct.

Do not guess about confidentiality
The rules require complainant identification. If investigated, the subject normally receives a copy unless doing so would jeopardize safety or a criminal investigation. Investigations are confidential with statutory exceptions and potential disclosure in disciplinary proceedings; confidentiality is not guaranteed anonymity.

Five-year office-discovery rule, with exceptions
RSA 310:9 generally requires allegations within five years after OPLC reasonably could have discovered the conduct—not five years from the patient's discovery or automatically from the incident. Exceptions include relevant criminal convictions or outside discipline and qualifying patterns connected to recent conduct. Plc 204 also directs reasonably prompt reporting; ask OPLC about your facts.

What may happen after filing
OPLC screens legal sufficiency, then investigates or recommends dismissal. Unfounded complaints are dismissed with written explanation. Supported matters can lead to disciplinary proceedings or, where appropriate, a confidential nondisciplinary concern letter.
