  • Use the Board’s complaint form. Give your contact details, the subject’s identifying information, a detailed statement and copies of relevant records, reports or correspondence.
  • Sign the statement before a notary. The form expressly says to contact a Board inspector if you cannot obtain notarization.
  • Return the form to the Board office, 5500 San Antonio Drive NE, Suite C, Albuquerque, NM 87109. The current homepage also directs documents to Pharmacy.Board@rld.nm.gov because its fax is not operational. Call 505-222-9830 with urgent patient or public-health concerns, then provide the complaint form.

Limits

The Board excludes ordinary drug prices and billing, nonfraudulent prescription insurance disputes, and malpractice by other healthcare practitioners, except pharmacist-clinicians. A substantiated violation of an enforced law or rule is required for license or registration sanctions.
