NYSED’s dentistry section includes dentists, dental hygienists and registered dental assistants. Office of Professional Discipline (OPD) handles professional misconduct and unlicensed practice. Describe the person’s treatment, records or conduct and identify the associated office; the practice’s name alone may not identify the responsible professional.

How the official source says to file

  • Complete and sign the Professional Discipline Complaint Form. Include the professional’s name, profession, business address, dates and specific events; attach copies of relevant bills or correspondence and keep originals.
  • Complete the records authorization, naming relevant practitioners or facilities. Sign and date it and obtain an adult witness’s signature and date. The witness must be at least 18; notarization is unnecessary. Leaving the authorization blank can delay investigation.
  • Mail the form to the OPD regional office closest to the incident, using the form’s directory, or fax it to 212-951-6420. Phone 800-442-8106 or email conduct@nysed.gov for information; complaints cannot be filed by telephone.

Limits

What this route may not provide

OPD cannot settle an ordinary disagreement about the price charged. It can investigate fraudulent billing and concerns about services not performed or performed poorly. Its enforcement process can impose license discipline; the checked instructions do not promise damages or a refund.

The paper form requests identification and a signature. Anonymous acceptance and withholding identity from the practitioner were not established. OPD says complaints that do not lead to discipline are confidential; disciplinary records become public.
