How the official source says to file
- Download the professional misconduct complaint form. Complaints must be written; the Office does not take them by telephone.
- Type or print in black ink. Describe what occurred, when, and where, and attach copies of relevant bills or correspondence. Keep original documents and preserve other physical evidence.
- Sign and date the complaint and send it to the appropriate regional Office of Professional Discipline address listed in the form. Complete the records authorization when applicable; it calls for your signature and an adult witness’s signature, without notarization.

The Office investigates and prosecutes professional misconduct; the Board of Regents makes final disciplinary determinations.
