OPD handles professional misconduct and unlicensed practice. New York’s pharmacy categories include pharmacists, registered pharmacy technicians, limited intern permits and pharmacy establishments; establishment verification covers in-state and nonresident pharmacies. Dispensing safety, counseling and professional conduct can be relevant. Identify the individual and pharmacy or business in the complaint.

How the official source says to file
- Complete and sign the complaint, describe what happened, when and where, and attach supporting copies. Retain original physical evidence, including incorrectly dispensed medication, in its original condition.
- Complete the records authorization and have an adult witness, age 18 or older, sign and date it. No notarization is required. Leaving authorization blank can delay investigation.
- Mail to the OPD regional office nearest the incident or fax 212-951-6420. Complaints must be written; telephone calls are for information. Call 800-442-8106 or email conduct@nysed.gov for help.

What this route may not provide
OPD does not resolve ordinary fee disputes or prices thought too high, but investigates fraudulent billing and may assist concerning work not performed or performed poorly. Medical malpractice suits use a separate process; physician and physician-assistant misconduct has a different regulator.

Do not guess about confidentiality
Complaints that do not result in discipline are confidential; disciplinary actions can be public. The form requests identity and signatures. Anonymous acceptance and limits on disclosure to the subject were not established.

Ask promptly about filing time limits
A filing deadline was not established by this check; ask promptly. OPD says almost all investigations finish within nine months; complex cases can take two years or more through final action. These are processing estimates, not filing deadlines.

An investigator gathers facts and contacts the complainant. An investigator, prosecutor and professional board member screen the case before possible prosecution; cases can close without action or produce discipline.

State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.
