How the official source says to file
- Read the complaint guidance, then open the Board’s Complaint Gateway. Identify the pharmacy and the personnel involved as accurately as you can; explain the dispensing event and supporting facts.
- Complete each portal section and upload supporting documents at the end. You can save an unfinished complaint using Cancel/Exit and retain its reference number.
- After submission, the form cannot be reopened or changed; contact the Board with additional information. The Complaint Coordinator at 919-246-1050 can help with filing. Later signature or authorization requirements were not established.

What this route may not provide
The Board cannot award compensation, demand an apology or resolve customer-service issues. It excludes insurance billing disputes, prior authorizations, insurance rejections and medication shortages. Its complaint page distinguishes prescribers’ prescribing practices from pharmacy regulation. A refusal to dispense may reflect the pharmacist’s professional judgment about safety or validity.
