North Dakota State Board of Pharmacy

Complaint instructions checked against the linked sources, with limitations and known conflicts. Confirm that the authority covers your concern.

Authority and scope

Check the license and concern

The Board accepts written complaints about pharmacies, pharmacists and pharmacy technicians. Name the pharmacy and individual involved, identify the patient and incident location, and describe specific conduct. The checked instructions do not separately explain intern complaints or the reach of jurisdiction over an out-of-state pharmacy; ask staff about those situations.

Submission

How the official source says to file

  • Submit the online complaint or print the paper form and mail it to the Board. Its current office address is 1838 E Interstate Avenue, Suite D, Bismarck, ND 58503; questions: 701-877-2404.
  • Provide complainant and patient information, the pharmacy or pharmacist, incident date and a specific narrative. Attach or upload copies of records, labels or prescription-container information; identify where other evidence can be obtained.
  • Sign the paper truth declaration, or check the online certification. The checked form does not request notarization.

Limits

What this route may not provide

The described process concerns disciplinary review. Refunds, damages, ordinary insurance disputes and prescribing complaints are not addressed in these instructions; do not assume this process can provide those remedies.

Anonymity

Do not guess about confidentiality

The Board generally sends the complaint to the pharmacist-in-charge, pharmacist or technician for a response. Investigative Committee deliberations are confidential, but hearings and Board meetings are open. The Board tries to protect patient identity where possible; this is not a promise of anonymous filing.

Ask promptly about filing time limits

A filing deadline was not established by this check; ask promptly. No fixed completion estimate was established.

Process

What may happen after filing

Expect acknowledgment, the subject’s response and a decision letter. The Investigative Committee may close the matter, resolve it administratively or pursue formal discipline. If a hearing occurs, you may be asked to testify; the Board provides hearing and final-order information.
