Check the facility type and concern
ODH’s Bureau of Survey and Certification supplies a consumer facility complaint form. CMS identifies Ohio’s survey contact and a separate home-health number. The form establishes an intake, not the complete list of Ohio state licenses or a universal route for every residential or addiction-treatment program; confirm the named facility’s oversight.

Match the route to the facility type
ODH consumer facility complaint
Readable survey complaint form; ask ODH to confirm the license/setting and separate behavioral-program routing.

- Use the ODH Bureau of Survey and Certification complaint form. It requests facility name/type/address, patient information and relationship, known wrongdoers, and a narrative with date, time, location and witnesses. The narrative is marked required; follow the form’s current required-field markings.
- Provide your contact details if you want acknowledgment and findings letters; anonymous reporting is permitted. Patient-record releases, representative authority and attachment requirements were not established; ask before sending another person’s records.
- For help or jurisdiction confirmation, CMS lists 800-342-0553 and, separately for home health, 800-669-3534. Ask whether the concern needs another Ohio licensing or treatment-program process.

What this route may not provide
Coverage for a particular hospital, surgical or outpatient facility, nursing home, home-health agency or hospice should be confirmed with intake. Assisted living/adult residential care and behavioral or addiction treatment may require different licensing units; this check did not establish their current routes. Do not assume federal certification covers every state-licensed facility, private office or residential program.

Do not guess about confidentiality
ODH’s form permits anonymity and says person-identifiable information is confidential. Anonymous reporters cannot receive follow-up or results. For complaints about a home under Ohio Revised Code 3721.031, identity protections have court-order, enforcement, proceeding and specified agency-sharing exceptions; they do not make the entire complaint file secret.

Ask promptly; consumer filing cutoff not established
No consumer filing cutoff or investigation/completion target was established from the readable form. Ask promptly; mandatory incident reporting and benefit/discharge appeals are separate processes.
What may happen after filing
The form offers acknowledgment and results letters to people who provide contact information. Detailed triage, investigation and completion rules remain unverified. Keep your report and any receipt; reading this form does not establish that a submission succeeds.

Prepare a reviewable submission
State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.

Other facility processes to consider
These processes have different purposes. The national guidance below does not establish this state’s filing requirements or change the verification level shown above.
Hospital grievance
For hospitals covered by CMS hospital Conditions of Participation, ask for the grievance contact and written procedure. The hospital must specify response timeframes and give a written decision describing its contact, investigation, results and completion date. These are hospital rules, not a single rule for every facility. You may contact the responsible State Survey Agency without first completing the hospital process.

State oversight
Match external oversight to the facility type and license. State Survey Agencies help CMS assess federal compliance; state licensing may cover different settings. Confirm jurisdiction rather than assuming one contact handles every clinic or residential program.

Medicare quality review and appeals
People with Medicare can ask their Medicare quality-review organization (BFCC-QIO) about eligible quality-of-care concerns. A complaint is different from a fast appeal when discharge or covered services may be ending too soon. Follow the Important Message from Medicare or Notice of Medicare Non-Coverage immediately; the notice identifies the contact and deadline. An inpatient-to-observation status change may also have a separate fast-appeal route; ask for its notice and current instructions. A routine grievance does not substitute for an appeal.

Long-term-care advocacy
Long-Term Care Ombudsmen advocate for residents of nursing homes, assisted living and similar residential settings. They help resolve concerns according to the resident’s wishes; they are not licensing enforcement. Identity disclosure generally requires informed consent, with court-order and specified incapacity exceptions. Ask how resident, representative and complainant consent applies.
