Authority and scope
Check the license and concern
The Board enforces pharmacy-practice and legal drug-distribution laws. It licenses pharmacists, interns and technicians, and regulates pharmacy and other drug-distribution businesses. Its terminal-distributor category includes pharmacies, hospitals and other sites holding dangerous drugs; its licensing materials separately recognize nonresident pharmacies. The complaint form permits identifying both a business and a person.

How the official source says to file
- Use the online complaint form. Required information includes complainant identity and contact details, incident date, and whether a business or person is involved.
- For a prescription issue, provide the requested prescriber, patient and drug information, plus the prescription number, fill date and label details when available.
- Describe the complaint, witnesses and other agencies contacted. The narrative limit is 8,000 characters. Further release or signature requirements were not established.

What this route may not provide
The Board expressly excludes financial or billing disputes, drug-pricing concerns and insurance matters. Naming a prescriber in its form does not establish authority over every aspect of that professional’s care. Explain the drug-law or pharmacy concern and ask staff to determine jurisdiction.

Do not guess about confidentiality
The complaint page says the complainant’s identity is confidential under Ohio law. Information may be shared with another investigative agency when jurisdiction belongs there, subject to the same confidentiality requirements. Required identifying fields mean this form should not be described as anonymous.

Ask promptly about filing time limits
A filing deadline was not established by this check; ask promptly. The checked consumer form does not promise a processing time.
What may happen after filing
The Board reviews the complaint’s issues and jurisdiction and may share information with a more appropriate investigative agency. Detailed consumer investigation stages and compensation powers beyond the stated exclusions were not established.
