Care first. Seek urgent medical or dental attention for serious or worsening symptoms. Researching or preparing a complaint should never delay treatment.

Boards of Behavioral Health Licensure, Examiners of Psychologists, Licensed Social Workers, and Licensed Alcohol and Drug Counselors

Psychology instructions checked; behavioral-board entry checked; social-work and addiction details limited. Some filing instructions were checked, but complete instructions for every listed credential have not been verified. Read the limits for your route.

Authority and scope

Check the license and concern

Behavioral Health Licensure covers LPCs, LMFTs, Licensed Behavioral Practitioners and its licensure candidates. Psychologists use the Board of Examiners of Psychologists. Social workers use the Board of Licensed Social Workers. Alcohol/drug credentials belong to a separate state board; the official behavioral-board page identifies it as the Oklahoma State Board of Licensed Alcohol and Drug Counselors. Do not treat every person called a therapist as a behavioral-board licensee.

Match the route to the credential

Use the license listed in the practitioner’s records or official license lookup. A psychiatrist is a physician; psychiatric nurses have nursing credentials. Some psychologists have prescribing authority, so medication alone does not identify the licensing board.

Submission

Checked steps and filing details to confirm

  • LPC, LMFT or Licensed Behavioral Practitioner: follow the Request for Inquiry link on the Behavioral Health Licensure complaint page. The referral and individual-license scope were checked; detailed portal fields, signatures and therapy-record releases remain unverified.
  • Psychology: submit a written, signed and notarized Request for Inquiry identifying the psychologist and factual allegations. The form says the attached narrative must also be signed and notarized. Include dates, harm, witnesses, relevant materials and requested action.
  • Mail the psychology packet to 421 NW 13th Street, Suite 180, Oklahoma City, OK 73103. The Board’s inquiry number is 405-522-1333. Its form asks whether you will give a sworn statement or testify.
  • Social work: the Board homepage links its complaint portal and says complaints from any source or form are reviewed. Current intake fields, authorized delivery alternatives, signatures and patient/representative consent were not established.
  • Alcohol/drug counseling: request the separate Board’s current complaint instructions. The checked behavioral-board guidance establishes the authority, not its filing method or confidentiality rules.

Limits

What this route may not provide

Behavioral Health Licensure regulates individuals, not facilities or agencies. Its guidance directs inpatient-facility concerns to the Health Department and outpatient behavioral-agency concerns to Mental Health and Substance Abuse Services. Refund or damages authority was not established. For a psychiatrist (MD/DO), use the medical guide; for a psychiatric advanced-practice nurse, use the nursing guide. Identify any facility or program complaint separately from the individual credential.

Anonymity

Do not guess about confidentiality

The psychology form permits copies to the psychologist and warns that complaint information may be supplied under public-records requests. It also warns that confidential clinical records introduced in a formal hearing can become public. Anonymous filing is incompatible with its stated signed/notarized procedure. Privacy and anonymity for the other boards remain unverified.

Process

What may happen after filing

Psychology may arrange an informal meeting or investigation. A probable-cause committee recommends dismissal or formal charges; the Board may approve a settlement or hold a hearing, where the complainant may be subpoenaed. The other boards’ detailed procedures remain limited.

Evidence

Prepare a reviewable submission

State the concern you want reviewed, give a short chronology and factual consequences, and explain your request. Keep supporting records and follow the receiving authority’s instructions before attaching or sending them.
