  • Call 405-521-3815 or email Pharmacy@pharmacy.ok.gov to discuss concerns with a compliance officer. Before investigation, the Board requires a written complaint; any written format is acceptable.
  • The optional form asks for the patient, drug name and strength, amount, prescription number, fill date, prescriber, pharmacy and pharmacist if known. Describe the incident, sign and date the truth affirmation; the form has no notary block.
