Denturists use the Health Licensing Office (HLO) and Board of Denture Technology; HLO’s form also lists dental laboratories. Dental assistants are not licensed in Oregon, but certain duties require certification through DANB for the Dentistry Board; ask that Board how to report an assistant-related practice or supervision concern.

  • Dentistry: complete the signed August 2026 form with the licensee, clinic, patient, treatment dates, other treating dentists and supporting copies. Mail it to 1500 SW 1st Avenue, Suite 770, Portland, OR 97201, or email information@obd.oregon.gov.
  • Denturists: use HLO’s July 2026 Consumer Complaint Form, select Denture Technologists, sign it and describe any later adjustments or work on the dentures and who performed it. Keep copies and submit all pages.
  • HLO accepts email to hlo.regulatory@odhsoha.oregon.gov, mail or in-person delivery to 1430 Tandem Ave. NE, Suite 180, Salem, OR 97301-2192. Its form also permits fax to 503-370-9216. Choose one method.

Limits

What this route may not provide

The Dentistry Board excludes ordinary fees, personality conflicts, rudeness and scheduling disputes. Its form does not guarantee damages. HLO expressly cannot require client refunds; it investigates violations and can impose licensing sanctions.

Anonymity

Do not guess about confidentiality

Dentistry allows anonymous complaints if enough information is supplied, protects identities as far as possible, and keeps investigations confidential; the licensee nevertheless receives the allegations for response. Proposed and final discipline is public. HLO’s form asks for a name and signature; anonymous acceptance was not established. HLO’s disciplinary notice becomes public, while staff restrict investigative disclosures.
