The Board regulates pharmacy practice and drug manufacture and distribution. Its categories include pharmacists, interns, pharmacy technicians and certified technicians, plus drug outlets and facilities. Facility licensing includes in-state and out-of-state retail or institutional pharmacies, central-fill and remote-processing outlets. Identify the individual and pharmacy or outlet involved so staff can assess the relevant credentials.

How the official source says to file

  • Use the Board’s online complaint form. Identify yourself, the patient, pharmacy, pharmacist if known and other involved personnel; give dates and a chronological account.
  • For dispensing errors, describe the medication’s appearance or imprint, incorrect label information and what should have appeared. Supply prescription and prescriber details, any patient harm, counseling received and discussions with pharmacy staff.
  • Retain prescriptions, receipts, correspondence and other paper evidence; the Board may request them. The form permits attachments and asks about willingness to testify. Limits

What this route may not provide

The Board’s consumer page expressly excludes authority over medication prices and customer-service issues. Its checked materials did not establish compensation powers or a route for insurance or PBM payment disputes. A pharmacy complaint does not automatically resolve those separate concerns.
