Care first. Seek urgent medical or dental attention for serious or worsening symptoms. Researching or preparing a complaint should never delay treatment.

Pennsylvania Department of State, Professional Compliance Office / State Board of Pharmacy

Pharmacy instructions checked; separate drug-industry route limited. Some filing instructions were checked, but complete instructions for every listed credential have not been verified. Read the limits for your route.

Authority and scope

Check the license and concern

The Pharmacy Board regulates pharmacies, pharmacists, technicians, technician trainees and interns, including unlicensed practice. Pennsylvania also registers nonresident pharmacies. It reviews professional standards and misconduct. Identify both the business establishment and individual involved using the complaint form’s separate fields.

Submission

Checked steps and filing details to confirm

  • Use the Department’s linked PALS complaint service or paper Statement of Complaint. Give event details, respondent information and witnesses; attach supporting copies, not original documents. Retain your complaint copy.
  • Sign the paper verification. Complete the records release if applicable and have the witness sign it; the checked form does not require a notary.
  • Mail the completed form to Professional Compliance Office, Department of State, PO Box 69522, Harrisburg, PA 17106-9522, or fax 717-705-2882. Call 717-783-4849 or email RA-ST-Complaints@pa.gov for form assistance.

Limits

What this route may not provide

The Department generally cannot obtain monetary restitution. Wholesalers, distributors and manufacturers fall under the Department of Health’s Bureau of Drugs, Devices and Cosmetics, according to the Pharmacy Board; this professional-licensing guide does not verify that separate complaint route.

Anonymity

Do not guess about confidentiality

The Department accepts information from all sources, but anonymous handling and disclosure during investigation were not established by these checked materials. Final disciplinary actions are public. If you later submit an impact statement, your identity and statement are public, the respondent gets a copy and you may be subpoenaed.

Complaint timing

Ask promptly about filing time limits

A filing deadline was not established by this check; ask promptly. Investigation time varies with complexity. The requested three-business-day lead for an impact statement concerns a later hearing, not initial complaint filing.

Process

What may happen after filing

The Professional Compliance Office screens jurisdiction and possible violations. Investigators may interview witnesses and collect documents; the licensing board adjudicates charges or approves consent agreements.
