Authority and scope
Check the professional and concern
The Board expressly accepts complaints about medical doctors (MDs), osteopathic physicians (DOs), and physician assistants. It also regulates several allied professions. Check the practitioner’s name and credential before filing. Hospitals, surgical centers, nursing homes, nurses, and other separately regulated professions are outside this Board’s listed complaint jurisdiction.

How the official source says to file
- Use the online Board complaint form linked from the official complaint page. Each submitted form must concern only one provider and your specific complaint against that provider.
- Prepare the provider’s correctly spelled name, relevant treatment dates and locations, and a factual account with available supporting documents. Keep your own copy before submitting.
- The complaint instructions were readable, but the embedded intake did not expose its fields during this check. Signature, record-release, attachment, and alternative delivery requirements were not verified. Ask the Board at 605-367-7781 for help if the online form is inaccessible.

What this route may not provide
The Board says it has no jurisdiction over disputes concerning fees. Its process addresses violations of professional laws and regulations and may result in compliance measures or other Board action. The checked instructions do not establish a damages or refund remedy for patients.

Do not guess about confidentiality
The Board states that investigation material is confidential by law. Its checked instructions do not establish whether anonymous complaints are accepted or whether identifying details are shared with the provider. Ask before relying on anonymity; a confidential investigation does not necessarily conceal the complainant from its subject.

A filing deadline was not established by this check.
Ask the Board promptly about your incident dates. The agency describes investigation as time-consuming but supplies no guaranteed completion time. That description is about processing, not a filing period or a court deadline.
What may happen after filing
The Board acknowledges receipt, checks jurisdiction, and decides whether investigation is warranted. It may seek additional information. A matter can close if no violation is found or lead to compliance or other action; a formal hearing can require witness testimony. The Board says it will notify you when review or investigation ends.
